Q: Could you provide our readers with a brief introduction to your company?
A: BlockForms is a web application that streamlines filling out tedious forms by consolidating forms that ask the same redundant questions into one digital form. This is a “fill out one form, populate many forms” time saver currently serving the insurance and investment industry.
Q: Any highlights on your recent announcement?
A: BlockForms is pleased to announce the launch of a private placement offering in accordance with Regulation Crowdfunding (Reg. CF) adopted by the U.S. Securities and Exchange Commission (SEC) through MicroVentures.

Q: Can you give us more insights into your offering?
A: BlockForms’ mission is to automate all forms. BlockForms’ form-automation software enables its customers to provide data via a single interface and have the information populate into all the requisite channels.
To date, the company has focused on building out its software and raised early investment from SAM Venture Partners to prepare for a wider product launch and a more aggressive outbound sales motion. Initial progress has resulted in BlockForms earning its first customers in 2021 and 2022 from GIS Benefits and Icarus Investment Group, respectively.
